Youth Agreement of Participation

By allowing my child to accept enrollment into programs offered by the Hartford Housing Authority Youth Division, I / we accept the conditions as set forth by this contract. The conditions for participation are as follows:

  1. I / We agree to ensure that my child will adhere to the rules and regulations of the program/s in which he/she is expected to participate

  2. I / We agree to ensure that my child participates fully cooperates with the program operation / s.

  3. I / We agree to ensure that my child will follow his / her service plan as outlined by the Youth Division.

  4. I / We understand that if my child does not cooperate and put forth the effort necessary to insure his / her success in following their service plan, it can jeopardize the family’s continuation in the Homeownership Program.

  5. This contract may be terminated by any of the following:

    • When the participant has failed to fulfill the terms of the contract

    • Withdrawal from the Program by the participant

    • Mutual consent of the parties

    • Actual Purchase of my unit under the homeownership program

NOTE: THIS AGREEMENT IS A PART OF THE HEAD OF HOUSEHOLD’S CAMPUS OF LEARNERS CONTRACT OF PARTICIPATION.

Criteria for participation YOUTH MUST BE:

  • Public housing resident (must be on the lease)
  • Have a minimum of a C average or be enrolled in a tutorial program
  • Have a completed application of participation on file
  • Not use or sell any illegal drugs or substance
  • Participate in study sessions as assigned
  • Attend school
  • Abstain from swearing
  • Not be involved with gangs
  • Respectful toward others
  • Uphold all rules and regulations regarding all Hartford housing programs

Failure to comply with these rules and regulations will result in your immediate termination from all youth programs and activities
